Forest Service Wildland Fire Management was notified through the Wildland Fire, Lessons Learned Center web site of incidents involving chainsaws and fuel tank vapor lock resulting in fires and employee injuries. The incidences reports were on August 7, 2008, July 1, 2013 and June 11, 2014. The U. S. Forest Service published an Incident Review Summary and Incident Review Presentation on March 15, 2015.
