29 CFR 1910.133(b)(1): Protective eye devices did not comply with any of the ANSI consensus standard: (a) Willow Peak Fire: On or about May 24, 2015 and times prior, employees were exposed to flying wood chips and burning embers while using a Stihl Chainsaw, Serial Number, 1 73 510 827 while attempting to cut down a burning tree with sunglasses that were not ANSI approved.

29 CFR 1960.30(a): The Agency did not ensure prompt abatement of unsafe and unhealthful conditions: (a) Willow Peak Fire: U. S. Forest Service Wildland Fire Management was notified through the Wildland Fire, Lessons Learned Center web site of incidents involving chainsaws and fuel tank vapor lock resulting in fires and employee injuries. The incidences reports were on August 7, 2008, July 1, 2013 and June 11, 2014. The U. S. Forest Service published an Incident Review Summary and Incident Review Presentation on March 15, 2015.
