OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to being struck by/crushed by falling equipment hoisted by a crane. LOCATION: 261 Madison Ave, New York, NY On or about May 31, 2015 A) The condensing section of a 11,000 pound HVAC unit was rigged by using 2 nylon web slings wrapped around the base of the unit . The slings were basket rigged and the padding at the edges/corner was insufficient.

LOCATION: 261 Madison Ave, New York, NY On or about May 31, 2015 A) Employees were engaged in lifting and loading several pieces of a large Heating Ventilation and Air Conditioning (HVAC) system to the 30th floor of a high rise commercial building. Inspection conducted prior to each lift of all rigging equipment and nylon slings were not properly documented and maintained.
