29 CFR 1926.150(c)(1)(i): A fire extinguisher, rated not less than 2A, was not provided for each 3,000 square feet of the protected building area, or major fraction thereof: At the project, there was no fire extinguisher onsite as it was in the truck of the owner who was not onsite. There were no alternate means to fight fires onsite, such as 55 gallon drums with water and pails or hoses connected to water sources. There were dry weeds around the site and a cut off saw with an abrasive wheel was used to cut steel all threads. Employees were exposed to the hazards of fire.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws: At the project, the outside insulation was pulled back from the receptacle on a flexible cord from a 3500 watt All Power generator to a 110 VAC Milwaukee cut off saw used to cut all thread. The all thread was used to hold down rebar from displacement during pouring of concrete. Employees were exposed to the hazards of contacting electricity.

29 CFR 1926.1053(b)(4): Ladders were not used only for the purpose for which they were designed: At the project, a four foot long step ladder was used in the folded up position to access the top of the rebar and formwork for the base of the water tank. Employees that used the ladder were exposed to the hazard of falling from a height of about 4 feet to the gravel below.
