29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:  Employees were observed working on a low slope roof approximately 12 feet above ground level without fall protection. Employees were not protected against a fall by a fall protection method or personnel fall arrest equipment. Employees were exposed to falls from the roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  At this renovation site, the ladder used to access the roof did not extend 36 inches above the access point.  Employees were exposed to falls of up to 10 feet.
