29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level: a) On the GOH receiving runway located outside the facility manager's office: On or about May 29, 2015 and at times prior thereto, the employer exposed employees to fall hazards of 10 ft. 6 in., in that platform railings included a vertical height of 24 inches, as maintenance personnel and others cleared garment jams and made visual inspections.

29 CFR 1910.27(e)(1): "Preferred pitch." The preferred pitch of fixed ladders were not in the range of 75 degrees and 90 degrees with the horizontal (fig. D-11): a) At the poly-bagger platform access area: On or about May 29, 2015 and at times prior thereto, the employer exposed workers to fall hazards of up to 8 ft., in that a fixed metal ladder was pitched at 70 degrees from horizontal, as maintenance workers accessed the upper platform to perform weekly and monthly preventive-maintenance activities, among other duties.
