29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about June 02, 2015 and at times prior thereto, employees were working on the roof of a residential building located at 14236 Earl Cholski, El Paso, TX 79938. They were using a ladder to access the second story of the building but the side rails did not extend at least three feet above the landing. This exposed the employees to fall hazards of more than six feet to the ground below.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: On or about June 02, 2015 and at times prior thereto, employees were working on a residential building located at 14236 Earl Cholski, El Paso, TX 79938. They were not properly using fall protection which exposed them to fall hazards of more than six feet to the ground below. Federico Urena was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 580138, citation number 1, item number 1 and was affirmed as a final order on February 13, 2013, with respect to a workplace located at 14353 Early Morn, El Paso, TX 79938.
