29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: On or about May 18, 2015, the bathrooms and general work area were not kept clean and orderly. a) The bathrooms were not clean and sanitary and there was standing water on the floor below one commode in the mens bathroom. b) Throughout the general work area; dirt, dust and miscellaneous debris was permitted to build up in and around employee work stations.
