29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about June 2, 2015, at the above addressed jobsite, employee was exposed to a 12 foot fall hazard when performing removal of plastic forms without means of fall protection.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  On or about June 2, 2015, at the above addressed jobsite, employee was exposed to a 12 foot fall hazard when performing removal of plastic forms from a window opening without means of fall protection.
