(a)Cordova Vicente, at 926 Coffman Street, Longmont, CO 80501: On and before May 12th, 2015, the exposing employer failed to provide his employees with adequate eye protection when employees where observed utilizing equipment which required it. An employee was observed utilizing a pneumatic nail gun without adequate eye protection while performing roofing work.

The condition exposed employees to fall hazards of approximately 12 feet.
