29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  On August 12, 2015, Speed Lube, LLC was issued citations and the employer did not verify, through written certification that each citation was abated.  Abatement certification was not received for the following violations:      a.    Citation 1 Item 1 and , b.    Citation 1 Item 2.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  Employees performing automotive service work were exposed to fall hazards of approximately 6 feet and the employer did not ensure the lower level service openings were covered or protected:  a. The bay 1 pit (southern most bay closest to office area) was open and a suitable method was not used to prevent employees from falling into the opening.  b. The bay 2 pit (north of bay 1) was open and a suitable method was not used to prevent employees from falling into the opening.  c. The bay 3 pit (north of bay 2) was open and a suitable method was not used to prevent employees from falling into the opening.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    Employees performing automotive service work were exposed to burns and smoke inhalation because extension cords were used a substitute for the fixed wiring of a structure:    a. The garage door opener used to raise and lower the exit door of bay 1 (southern most bay next to office) used an extension cord to permanently power the opener.    b. The garage door opener used to raise and lower the exit door of bay 3 used an extension cord to permanently power the opener.    c. The garage door opener used to raise and lower the exit door of bay 4 (northern most bay furthest from office) used an extension cord to permanently power the opener.
