29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Employees were observed walking and working on the ceiling joists of a residential structure at heights greater 6 feet without the use of fall protection, exposing employees to fall hazards.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  Employees framing on a residential structure were exposed to greater than 6 feet fall hazards from a frame-out window opening, which lacked guarding.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     Employees were ascending and descending from the second level of a residential home under construction on an aluminum ladder which did not extend three feet above the upper landing, which exposed the employees to the hazard of stumbling and falling from heights greater than 6 feet to the ground below.
