29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that posed a hazard by falling or rolling into excavations and protection was not provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: (a) University of Mississippi campus, Proposed Alpha Delta Pi Sorority House, 607 Rebel Drive, University, Mississippi 38677 - On or about May 28, 2015, employees were installing/laying 10 foot x 4 inch diameter joints of electrical duct and spacers-supports for a transformer pad at this location in a trench approximately 85-90 feet long by 15-16 inches to ~2 feet wide at its bottom by 2.5-4.5 feet deep in Class C soil, with the spoil piles ranging from 3.3 to 4.67 feet above ground level at the trench's edges on both the north and south sides.

29 CFR 1926.652(a)(1)(ii): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph 29 CFR 1926.652(b) or 29 CFR 1926.652(c) of this section except when: excavations are less than 5 feet (1.52 m) in depth and examination of the ground by a competent person provides no indication of a potential cave-in. (a) University of Mississippi campus, Proposed Alpha Delta Pi Sorority House, 607 Rebel Drive, University, Mississippi 38677 - On or about May 28, 2015, employees were exposed to cave-ins while installing 10 foot long x 4 inch diameter joints of electrical duct and spacers-supports in a trench approximately 85-90 feet long by 15-16 inches to ~2 feet wide at its bottom by 2.5-4.5 feet deep in Class C soil, with the spoil piles ranging from 3.3 to 4.67 feet above ground level at the trench's north and south edges.
