29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:       a) 935 Amelia Street, Belle Vernon, PA 15012:  Observed on or about June 03, 2015, the spoil pile from a trench was not placed at least 2 feet from the edge of the trench.  An employee working at the bottom of the trench was exposed to struck-by and engulfment hazards while installing a 4 inch diameter sewer line at the bottom of the trench.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    a) 935 Amelia Street, Belle Vernon, PA 15012:  Observed on or about June 03, 2015, an employee working at the bottom of a trench that was approximately 9 feet deep with near vertical sidewalls was not protected from cave-ins.  The employee was exposed to an engulfment hazard while installing a 4 inch diameter sewer line located at the bottom of the trench.

29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in a possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions had been taken to ensure their safety:       a) 935 Amelia Street, Belle Vernon, PA 15012:  Observed on or about June 03, 2015, the competent person did not remove an employee from a trench that was approximately 9 feet in depth with near vertical sidewalls when hazardous conditions existed.  The employee was not protected from cave-ins, the spoil pile was not set back at least two feet from the edge of the trench, and a ladder used as a means of egress was not long enough to reach over the tope of the trench by at least three feet.  The employee was exposed to struck-by, fall, and engulfment hazards while installing a 4 inch diameter sewer line located at the bottom of the trench.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) 935 Amelia Street, Belle Vernon, PA 15012:  Observed on or about June 03, 2015, a portable ladder used as a means of access/egress in a 9 foot deep trench did not extend at least 3 feet above the top of the trench.  The employee using the ladder as a means of trench access/egress was exposed to a fall hazard of approximately 9 feet to the bottom of the trench.
