29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated On June 3, 2015, an employee was not protected from a 25 foot fall hazard while operating an aerial lift and the employer did not ensure employees were utilizing fall protection. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On June 3, 2015, an employee was not protected from a 25 foot fall hazard while walking/working on the roof of a commercial building and the employer did not ensure fall protection was utilized. B Chris Construction, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(1) or its equivalent standard which was contained in OSHA inspection number 898877, citation number 01, item number 01 and was affirmed as a final order on May 22, 2013, with respect to a workplace located at 736 Forest Ave. River Forest, IL 60305. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
