29 CFR 1926.502(h)(1)(v): The safety monitor shall not have other responsibilities which could take the monitor's attention from the monitoring function: a. 206 South Avenue, Fanwood, NJ - Provident Bank Employees engaged in roofing repairs of a low pitch roof were not protected from falls by use of guardrail systems, safety nets systems, personal fall arrest systems, or a warning and safety monitor system. Employees accessed the roof by walking across an open edge awning (within 6 foot of the edge), approximately 13 feet over the driveway of the bank building. Violation observed on or about June 3, 2015 The Pieros Construction Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 314681289, citation number 1, item number 2 and was affirmed as a final order on March 13, 2012 , with respect to a workplace located at 231 Main St., Little Falls, NJ.
