29 CFR 1910.1200(h)(1): Employer has not provided employees with effective information and training on hazardous chemicals in their work area at the time of initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area:    a) United Continental Holdings, Inc. DBA United Airlines at 8900 Pena Blvd., Denver, CO: On and before 06/02/15, the employer did not ensure that employees were provided effective information and training as specified in 29 CFR 1910.1200(h) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area. Employees used a floor grinder to prep concrete floors for sealing and then applied an epoxy paint sealant. Employees were not provided information or training on the chemicals used.  Employees were exposed to hazards associated with chemicals such as but not limited to:    1.  Silica Dust (created by grinding on concrete floors);  2.  Isocyanates (Rust-Oleum Consav 1-GLK Fastkote UV Light Gray floor coating, containing 5% HDI).

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace:   (a) United Continental Holdings, Inc. DBA United Airlines at 8900 Pena Blvd., Denver, CO: On and before 06/02/15, the employer did not identify and evaluate the respiratory hazard in the workplace to determine accurately the airborne concentrations of silica dust to which employees may be exposed. The employer did not determine employee exposures during operations associated with grinding on concrete floors. This condition potentially exposed employees to respiratory hazards associated with exposure to crystalline silica.  (b) United Continental Holdings, Inc. DBA United Airlines at 8900 Pena Blvd., Denver, CO: On and before 06/02/15, the employer did not identify and evaluate the respiratory hazard in the workplace to determine accurately the airborne concentrations of hexamethylene diisocyanate (HDI) to which employees may be exposed. The employer did not determine employee exposures during operations associated with applying epoxy paint containing hexamethylene diisocyanate (HDI). to concrete surfaces. This condition potentially exposed employees to respiratory hazards associated with isocyanates.
