29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:      (a)   Bulldog Renovations, LLC dba G.J. Gardner Homes-Denver at 3335 W 20th Ave, Denver, CO 80211:  On and before May 28, 2015 the controlling employer did not ensure that subcontractor employees were protected from injury, in that two subcontractor employees framing the top of the three-story residential home were not utilizing a fall protection system.  This condition exposed the subcontractor employees to a fall hazard of approximately 10 feet to the lower level.      (b)  Bulldog Renovations, LLC dba G.J. Gardner Homes-Denver at 3335 W 20th Ave, Denver, CO 80211:  On and before May 28, 2015 the controlling employer did not ensure that subcontractor employees were protected from injury, in that one subcontractor employee framing near the edge on top of the second level was not utilizing a fall protection system.  This condition exposed the subcontractor employee to a fall hazard of approximately 20 feet to the ground level.     Bulldog Renovations, LLC dba G.J. Gardner Homes-Denver was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 899316, Citation Number 1 Item 1b issued on July 19, 2013, and became Final Order on August 22, 2013, with respect to a workplace located at 3935 Utica Street, Denver, CO 80212.
