29 CFR 1926.417(b): Equipment and circuits. Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized: At the project, the circuits that were deenergized were not rendered inoperative as the wires were not removed from the circuit breakers that controlled the circuits, and tags were not attached to each circuit breaker that could energize the circuits. The circuits were located in rooms located adjacent to the project in panels FLK21 and FL2J. Employees that demolished the bar area with pry bars, hammers, and other tools were exposed to the hazard of contacting electricity.
