Instances #2:  At the Auto Loader, the employer did not provide machine guarding to protect the forklift operator and other employees from rotating parts at the side of the stacked pallet where the operator loads the pallet.       Instances #3:  The employer did not provide machine guarding to protect the operator from the point of operation on the polar cutter when light curtains and two hand trip controls did not prevent operators from reaching the cutting blade.
