29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Instances #1: The employer did not provide machine guarding to protect the operator and other employees from pinch points between a stationary polar cutting table and rotating tables (such as Schneider Auto Loader, Model #3030, Number 8119-1462). Instances #2: At the Auto Loader, the employer did not provide machine guarding to protect the forklift operator and other employees from rotating parts at the side of the stacked pallet where the operator loads the pallet. Instances #3: The employer did not provide machine guarding to protect the operator from the point of operation on the polar cutter when light curtains and two hand trip controls did not prevent operators from reaching the cutting blade.
