29 CFR 1910.26(c)(2)(iv): Ladder(s) were not maintained in good usable condition at all times: (a) Albertson, LLC, Store #881 @500 E. 120th Ave., Northglenn, Colorado. On or before May 29, 2015, as part of injury/accident prevention under its safety program, the employer did not ensure that each stepladder being used by employees for stocking shelves in the back room was maintained in good and serviceable condition. A stepladder had the top step defective. The base section of a stepladder was missing anti-slip rubber base covers to provide secure footing when in use. This condition exposed employees a slip/fall hazard when using the stepladders.

29 CFR 1910.178(q)(1): Powered industrial truck(s) with defect(s) or in any way unsafe had not been withdrawn from service until restored to safe operating condition(s). All repairs shall be made by authorized personnel. (a) Albertson, LLC, Store #881 @500 E. 120th Ave., Northglenn, Colorado. On or before May 29, 2015, as part of injury/accident prevention under its safety program, the employer did not ensure that the Baker Forklift, Model BWS40 was free from defect(s) or any way unsafe and removed from service until restored to safe operation condition and repairs made by authorized personnel. A T-handle Allen wrench had been used in lieu of the appropriate manufactures lift lower control handle. The valve set screw for the lift lower handle had been stripped out and personnel had been using the forklift in this condition for many months. The forklifts data plate was unreadable and not securely attached to the truck. These conditions exposed the employees to the hazards of an unsafe forklift operation.
