29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   a. 2165 Shackamaxon Dr., Scotch Plains, NJ    Employees accessed a Werner, Pump Jack scaffold platform which was more than two feet below the access level without the use of a ladder.  Violation observed on or about June 4, 2015

Employees were observed installing siding from a scaffold which was not secured at the bottom, thereby exposing employees to an unstable work platform and to fall hazards. Violation observed on or about June 4, 2015  Allaico Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 315642223, citation number 1, item number 1, and was affirmed as a final order on November 21, 2012, with respect to a workplace located at 971 US Highway 9, Building 4, Sayerville, NJ, 08872.
