29 CFR 1910.147(f)(1)(v): Deenergize all systems and reapply energy control measures to continue the servicing and/or maintenance. On or about June 3, 2015, the employer did not ensure that the towel folding machine was deenergized when aligning the belt. An employee was aligning the newly installed belt while the machine was running. This condition exposed the employee to caught-between hazards.
