In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1030(h)(1)(ii)(B): The employer did not establish and maintain an accurate record for each employee with occupational exposure which includes a copy of the employee's hepatitis B vaccinations and any medical records relative to the employee's ability to receive vaccination as required by paragraph (f)(2) of the OSHA Bloodborne Pathogens Standard.  a) Brickyard Dental Center & Clinic, P.C., Chicago, IL, did not maintain accurate and complete records of each employee's hepatitis B vaccination status, with the dates of all the vaccinations, for each employee with occupational exposure to bloodborne pathogens that consented to and/or was provided the hepatitis B vaccinations.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
