Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling: a) On or about 05/28/15, at 1579 Ave Ponce De Leon, San Juan, PR, in the CRC workstation area, a relocatable power tap was not directly connected to a permanently installed branch circuit receptacle. b) On or about 05/28/15, at 1579 Ave Ponce De Leon, San Juan, PR, in the SR1 workstation area, an uninterruptible power supply was not directly connected to a permanently installed branch circuit receptacle. c) On or about 05/28/15, at 1579 Ave Ponce De Leon, San Juan, PR, in the FE12 workstation area, an uninterruptible power supply was not directly connected to a permanently installed branch circuit receptacle. d) On or about 05/28/15, at 1579 Ave Ponce De Leon, San Juan, PR, in the FE13 workstation area, an uninterruptible power supply was not directly connected to a permanently installed branch circuit receptacle. e) On or about 05/28/15, at 1579 Ave Ponce De Leon, San Juan, PR, in the FE15 workstation area, an uninterruptible power supply was not directly connected to a permanently installed branch circuit receptacle.
