On or about June 4, 2015, The employer did not provide adequate machine guarding for the Rotary Die Cutter Machine, number 131, thereby exposing employees to cuts, lacerations, and amputation hazard(s).    The Welch Packaging Chicago Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1001045, citation number 01, item number 01, and was affirmed as a final order on February 24, 2015, with respect to a workplace located at 1000 N. Main Street, Lombard, IL 60148.
