29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a.  At the workplace, employees operated a Strip Technology, Inc., Model 5000, Serial # 8423, wire stripping machine that did not have the point of operation guarded on the small wire discharge side, exposing employees to a caught-in hazard.  On June 4, 2015, an employee received serious injuries from being caught-in the point of operation.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  a.  At the workplace, at the time of inspection, access to a shear machine electrical disconnect on the south wall of the processing building was blocked by cardboard boxes of scrap metal.   b.  At the workplace, at the time of inspection, access to a shear machine electrical disconnect on a column in the center of the processing building was blocked by cardboard boxes of scrap metal.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:  (A): By location in a room, vault, or similar enclosure that is accessible only to qualified persons; (B): By suitable permanent, substantial partitions or screens so arranged so that only qualified persons will have access to the space within reach of the live parts. Any openings in such partitions or screens shall be so sized and located that persons are not likely to come into accidental contact with the live parts or to bring conducting objects into contact with them;  (C): By placement on a suitable balcony, gallery, or platform so elevated and otherwise located as to prevent access by unqualified persons; or  (D): By elevation of 2.44 m (8.0 ft) or more above the floor or other working surface.   a.  At the workplace, in the processing building, a Quality Products International, Inc., model V6030, serial # 97101447, vertical bailer machine, had live electrical wires approximately six feet from the ground on the side of the bailer, that were not guarded against accidental contact.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  a.  At the workplace, a shear machine on the south wall of the processing building had a flexible cord showing visible damage and was not connected to a device and fitting so that strain relief was provided.

Located at Harry Masser Company, Inc. dba Masser Metals and Recycling in Columbus, Ohio:  a.  On the 2015 OSHA 300 Log, for the injury that occurred on June 4, 2015, the employer did not provide all information required in Column F such as the actual injury.
