29 CFR 1926.960(c)(2)(i): When an employee used rubber insulating gloves as insulation from energized parts (under paragraph (c)(1)(iii)(A) of this section), the employer did not ensure that the employee also used rubber insulating sleeves: (a) At the site - located at 619 Island Way, Clearwater, FL when conduit was installed in the energized 7200 volts transformer box, insulated sleeves were not provided exposing the employee to an electrical hazard, on or about 05/28/2015.
