29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:         (a) On or about June 3, 2015 at worksite 601 Conley, Columbia, Missouri.-employees were being required to wear Cordova NX59V Particulate respirators when using Target stone saw and there was no written respiratory protection program.  Employees were not provided a medical evaluation prior to being required to use the  Cordova NX59V Particulate respirators.          Abatement certification is required for this item - use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
