College Ave., Fort Collins, CO 80524: On and before May 14, 2015, the employer did not ensure that the dual wheel bench grinder located in the shop had the correct distance between the wheel periphery and the edge of the tool rest in the front and did not exceed 1/8 inch because the tool rest was missing. This condition exposed the employees to abrasion hazards to the hands.

College Ave., Fort Collins, CO 80524: On and before May 14, 2015, the employer did not ensure that several metal electrical outlet boxes were used in accordance with listed instructions, in that the outlet boxes in use on the flexible cords included pre-punched knockouts that would be dislodged by frequent handling. This condition exposed employees to electrical hazards while connecting and disconnecting electric-powered equipment at the outlet box at the end of a flexible cord.
