29 CFR 1910.215(a)(4):  Grinding machinery was not used with work rest(s) to support offhand grinding work:      (a)  Big O Tires at 1506 N. College Ave., Fort Collins, CO 80524:  On and before May 14, 2015, the employer did not ensure that the dual wheel bench grinder located in the shop had the correct distance between the wheel periphery and the edge of the tool rest in the front and did not exceed 1/8 inch because the tool rest was missing. This condition exposed the employees to abrasion hazards to the hands.

29 CFR 1910.215(b)(9):  Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:       (a)  Big O Tires at 1506 N. College Ave., Fort Collins, CO 80524:  On and before May 14, 2015, the employer did not ensure that the dual wheel bench grinder located in the shop had the correct distance between the wheel periphery and the end of the peripheral member at the top and did not exceed ¼ inch because the adjustable tongue was missing. This condition exposed the employees to the hazard of flying chips and debris.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:       (a)  Big O Tires at 1506 N. College Ave., Fort Collins, CO 80524:  On and before May 14, 2015, the employer did not ensure that several metal electrical outlet boxes were used in accordance with listed instructions, in that the outlet boxes in use on the flexible cords included pre-punched knockouts that would be dislodged by frequent handling. This condition exposed employees to electrical hazards while connecting and disconnecting electric-powered equipment at the outlet box at the end of a flexible cord.
