29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:    (a) The employer did not submit abatement documentation and certification for Citations 1 Item 1, Citation 1 Item 2, Citation 1 Item 3 and Citation 1 Item 4 for lack of protection from falls, lack of fall protection training, break in elevation greater than 19 inches, and lack of securing of portable ladders, respectively.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    A Werner step ladder used to access the top of the walls did not extend at least 3 feet above the upper landing nor was it secured with a grasping device provided.
