29 CFR 1926.405(b)(1): Openings in boxes, cabinets, and fittings through which conductors entered were not effectively closed: a) At the building construction site, on or about June 04, 2015, employees were exposed to electrical shock hazards of 120 volts and higher, from four electrical enclosures that had unused opening such as but not limited to unused breakers that were not effectively closed, while turning breakers on and off to trouble shoot installed systems such as but not limited to lighting. b) At the building construction site, on or about June 04, 2015, employees were exposed to electrical shock hazards of 120 volts and higher, from damaged electrical conduit that was running into the breaker enclosures and were not protected, from abrasion while turning breakers on and off to trouble shoot installed systems such as but not limited to lighting.
