29 CFR 1910.119(d)(3)(i)(D): The employer did not maintain an accurate listing of the location, function, and inspection information for all Relief Valves (RV) within a covered process. The employer did not document the relief system design and design basis for the ammonia refrigeration system. Employees working within the engine room and throughout the facility were potentially being exposed to explosion hazards and/or ammonia leaks as relief systems/valves were not designed and sized properly. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. The employer did not document that equipment complied with recognized and generally good engineering practices in related to the following: b) The employer did not assure the Oil Separators such as, but not limited to, VS130, VS131, VS132, VS133, VS138 where equipped with relief valves and had complied with applicable recognized and generally acceptable good engineering practices such as, but not limited to, ANSI/ASHRAE Standard 15-2010 Safety Standard for Refrigeration Systems. c) The following doors and/or areas were not tight-fitting to prevent the migration of ammonia into other work spaces in an event of an ammonia release: the door leading from production into the engine room, the housing of the refrigeration equipment (ex. compressors, etc.), the maintenance department. Also, door seals below and above (transom) doors such as, but not limited to, offices, the Dry Powder Room and Dry Parts Room on the second/upper level. The employer did not document compliance for each area within recognized and generally acceptable good engineering practices such as, but not limited to, ANSI/ASHRAE Standard 15-2010 Safety Standard for Refrigeration Systems. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
