29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On the roof of the building under construction, at least one employee working on the HVAC system was not protected from falling more than 17 feet to the lower level.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: On the west side of the building under construction, at least one employee using a wooden wire spool to access the bottom rung of the fixed ladder of the building where there was a break in elevation of 4 feet 11 inches was exposed to a fall hazard.
