29 CFR 1904.39(b)(5) The employer did not report a work-related fatality or in-patient hospitalization caused by a heart attack. The local OSHA Area Office director will decide whether to investigate the event, depending on the circumstances of the heart attack. On or about June 4, 2015: A fatality occurred when an employee working in his office on the sixth floor of the facility died of an apparent heart attack. The employer did not report the fatality.
