29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a) 1032 South St., Pittsfield, MA: On or about June 5, 2015 employees were working from raised aerial lifts to apply EIFS panels, more than 40 feet above the ground below without being attached to the boom or basket.     Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).    Gheorghe Mihai d/b/a  MG Stucco was previously cited for a violation of this occupational safety and health standard 29 CFR 1926 453 (B) (2) (iv) which was contained in OSHA inspection number 550018, citation number 1, item number 1 and was affirmed as a final order on Sept 7, 2012, with respect to a workplace located at 2750 Mosside Blvd, Monroeville, PA.
