Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a.) Russell Roofing Company, Martinsburg WV VAMC: On or about 5/01/15, employees performing roofing activities on a low slope roof were not protected from falling approximately 34.5' to the ground. ABATEMENT DOCUMENTATION IS REQUIRED.
