OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion and struck-by hazards from improper handling and filling of liquefied petroleum gas tanks and cylinders: Employees were not properly trained nor was training documented in accordance with the National Fire Protection Association 2008 LP-Gas Code Handbook section 4.4 identifying the requirements for the qualification of personnel who transfer, transport, or whose primary duties fall within the scope of the code.
