29 CFR 1926.403(b)(1)(iii): Safety of equipment was not determined on the basis of electrical insulation:      On or about June 05, 2015 and at times prior thereto, employees were working on a residential building and were using a circular saw with damage to its cord insulation. The extension cord used to connect to the saw also had insulation damage. This exposed employees to electric shock hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws:    On or about June 05, 2015 and at times prior thereto, employees were working on a residential home and they were using a Bull Rolair compressor which had strain relief damage. This exposed employees to electrical shock hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      On or about June 05, 2015 and at times prior thereto, employees were working on a residential building. One employee was working on a walking/working surface without any form of fall protection. The employee was exposed to fall hazards of more than six feet to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    On or about June 05, 2015 and at times prior thereto, employees were working on a residential building. One employee was using a step ladder as an extension ladder which exposed him to fall hazards of greater than six feet.
