29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about May 26, 2015, and times prior to, in the Fabrication Shop, the feed rollers on the Webb Plate Bending Roller machine were not guarded. This condition exposed employees to contact with nip points. Newberry Tanks & Equipment, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 940990, citation number 1, item number 3 and was affirmed as a final order on March 5, 2014, with respect to a workplace located at 205 North Walker Avenue in West Memphis, Arkansas.
