Employees were using the scaffold as a work platform to install Hardy Board to the apartment building.

Scaffold frame was used as a means of access. On or about June 4, 2015, at 155 Dr. Michael DeBakey Drive, Lake Charles, LA, the employer failed to ensure that employees used a ladder to gain access to the working levels of the scaffold system. Employees used the scaffold frame as a means of access to the working level.
