29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) King Soopers Bread Plant, 60 Yuma Street, Denver, CO: On and prior to November 24th, 2015, the employer did not ensure appropriate conveyor guarding of the ingoing nip points going toward and away from the East Pan-O-Mat machine. Condition exposed employees to amputation and/or abrasion hazards. (b) King Soopers Bread Plant, 60 Yuma Street, Denver, CO: CO: On and prior to November 24th, 2015, the employer did not ensure appropriate conveyor guarding of the ingoing nip points going toward and away from the West Pan-O-Mat machine. Condition exposed employees to amputation and/or abrasion hazards. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter").

29 CFR 1910.212(a)(2): Machine guard(s) on machine(s) were so installed that they offered accident hazards in themselves: (a) King Soopers Bread Plant, 60 Yuma Street, Denver, CO: On and prior to May 29th, 2015, the employer did not ensure that the access opening to the dough guides on the east Pan-O-Mat machine was effectively guarded, in that an employee sustained a serious hand injury after they opened the hinged guard and placed their hand inside the machine while it was operating. This condition exposed employees to amputation and/or crushing hazards while adjusting dough guides. (b) King Soopers Bread Plant, 60 Yuma Street, Denver, CO: On and prior to May 29th, 2015, the employer did not ensure that the access opening to the dough guides on the west Pan-O-Mat machine was effectively guarded, in that employees open the hinged guard and place their hands inside to make minor adjustments. This condition exposed employees to amputation and/or crushing hazards while adjusting dough guides. Abatement Note: Abatement certification is not required for this item.
