29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s). a) On or about May 28, 2015, in the Wire Subassemblies department at Delta Systems Inc. located at 1734 Frost Rd. Streetsboro, Ohio 44241, employees were able to reach into the point of operation created by the terminal crimping die on the Artos Wire Machine while the machine was fully operating and cycling.
