29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: On June 8, 2015, employees installing a sewer line entered an excavation between 8 and 12 feet deep and the employer did not ensure a competent person conducted an inspection of the excavation that included at least one manual soil classification analysis method, identified previously disturbed soil and the soil type and ensured the sloping and benching system used was appropriate for the excavation.
