29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation; On or about June 8, 2015, the employer failed to ensure that adequate precautions had been taken for water accumulating inside an excavation in which employees were performing a natural gas valve replacement. The hazard was present with respect to a worksite located at the intersection of State Route 5 and State Route 193 in Johnston Township, OH.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): On or about June 8, 2015 the employer failed to ensure that employees working in an excavation were protected from cave-ins. At a worksite located at the intersection of State Route 5 and 193 in Johnston Township, OH, employees were replacing a natural gas valve in an unprotected excavation 64-69' indepth, 12.5" wide, 9' in length, and class c type soil. Cave-in protection was not provided.
