29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) 208 Woodruff Street, Avenel, NJ 07001: Employees were observed performing siding operations and walked from the rear of the house to the front to dispose of garbage at a height of approximately 30' to the ground below with no fall protection in place, exposing them to fall hazards. Condition observed on or about June 4, 2015.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) 208 Woodruff Street, Avenel, NJ 07001: Employees were allowed to perform exterior siding operations approximately 30' from the ground below on an Alum-a-Pole pump jack scaffold without fall protection. Condition occurred on June 4, 2015. Mike's Carpentry Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i), which was contained in OSHA inspection number 313984825, citation number 01, item number 04 and was affirmed as a final order on 03/15/2011, with respect to a workplace located at 2800 New Brunswick Avenue - Building #6, Piscataway, NJ 08854.
