29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a)  At the jobsite, on the scaffold, on or about 6/3/15:  An employee was working on a ladder jack scaffold placed on the West side of the building, where the working level was approximately 18 feet from ground, and where no fall protection was provided.    b)  At the jobsite, on the scaffold, on or about 6/3/15: An employee was working on a ladder jack scaffold placed on the East side of the building, where the working level was approximately 18 feet from ground, and where no fall protection was provided.

29 CFR 1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall.    a)  At the jobsite, on or about 6/3/15:  An employee was working on an extension ladder, approximately 16 to 18 feet from ground, removing a section of shingle from the eave of the roof, when he fell and died as a result of that fall.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  a)  At the jobsite, on or about 6/3/15:  An employee was working on an extension ladder, approximately 16 to 18 feet from ground, removing a section of shingle from the eave of the roof, when he fell and died as a result of that fall and was not provided training on the safe use of ladders.      Abatement documentation must be submitted for this item.
