29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   (a) Roof of addition to facility located at 2 Wilbanks Road, Rome, Georgia. Employees using pneumatic nail guns, without eye protection, while installing roof sheathing were exposed to eye injuries from flying debris.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   (a) Roof of addition to facility located at 2 Willbanks Road, Rome, Georgia. Employees were not protected from falling approximately 11-feet to ground level while installing roof sheathing from an unprotected 4 in 12 pitched roof. The employees were not wearing personal fall arrest systems nor had guard rails or safety nets been installed to provide protection from falling.
