29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about June 4th 2015, at the jobsite located at NW 107th Ave with 74th St, House # 8/5, an employee was removing work form without the use of fall protection, being exposed to a fall hazard of approximately 14 feet.   EMME Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926. 501(b)(13), which was contained in OSHA inspection number 1039307, citation number 1, item number 1 and was affirmed as a final order on 04/01/15, with respect to a workplace located at 5637 SW 2nd St. Miami, FL 33134.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 4th 2015, at the jobsite located at NW 107th Ave with 74th St:  1. On the house # 8/5 located at 10560 NW 67th Ter., Employees were accessing the working area of the roof deck with a metal extension ladder that did not extend at least 3 feet above the landing, exposing the employees to a fall hazard of approximately 14 feet.  2. On the house # 2/6 located at 10581 NW, Employees were accessing the working area of the roof deck with a metal extension ladder that did not extend at least 3 feet above the landing, exposing the employees to a fall hazard of approximately 13 feet.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  On or about June 4th 2015, at the jobsite located at NW 107th Ave with 74th St, employees were accessing the working platform of the roof deck with a metal extension ladder, without being trained on how to use the ladder, exposing the employees to fall hazard of approximately 14 and 13 feet.
