29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) On or about June 9, 2015, at worksite 10245 Hickman Dr, Kansas City, Missouri.- employees were exposed to serious caught-in, crushing and or struck-by injuries, in that forklift operators operating a Raymond Reach rider side Forklift # 7451, had not completed training demonstrated by the successful completion of training and evaluation required by this standard. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

(a) On or about June 9, 2015 at worksite 10245 Hickman Dr, Kansas City, Missouri.-employees in the Packing warehouse area were exposed to struck by hazards from the Raymond Reach Rider Side Forklift # 7451 that was in operation, in that the horn was inoperative documented by multiple daily inspection forms. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
