29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a)Otero's Framing LLC at 10831 Unity Parkway, Commerce City, CO 80249:  On and before May 30th, 2015, the employer did not ensure the use of safety glasses when employees were using or working near pneumatic nail guns when framing this single family home.  The condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  (a)Otero's Framing LLC at 10831 Unity Parkway, Commerce City, CO 80249:  On and before May 30th, 2015, the employer did not ensure the use of fall protection when employees were framing this two-story single family home in the following instances:  1)  Employees were framing walls on the second floor immediately adjacent to the edge without the use of fall protection;  2)  Employees were framing on the second floor adjacent to an open stairway floor hole, with fall hazards to the basement; and  3)  Employees were framing on the first floor adjacent to an open stairway floor hole with fall hazards to the basement.  The condition exposed employees to fall hazards of up to 20 feet.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  (a)Otero's Framing LLC at 10831 Unity Parkway, Commerce City, CO 80249:  On and before May 30, 2015, the employer allowed the employees to use a Power Sentry electrical multi-tap, model 104, serial number E110241 34u8, with six receptacles, to be used outside, even though the manufacturer stated that it was for indoor use only.  This multi-tap was used to power multiple circular saws and a sawzall.  The condition exposed employees to electrical shock hazards on a 120 volt circuit.
